Postponed: Memorial and Jefferson Vaccine Clinic on Nov. 23

 

We have been notified by Walmart that they need to cancel our vaccination clinic scheduled for tomorrow, November 23 due to staffing issues. We are working to reschedule both dates and we will notify families as soon as we can confirm this.

Memorial High School will host a Covid Vaccine Clinic Tuesday, Nov. 23 and Tuesday, December 14 from 2-6 p.m. in room 620. Students and staff from Memorial and Jefferson Middle School with all completed forms are eligible. Vaccines will also be available for staff, both initial and booster doses. All forms must be completed for vaccination. The forms can be submitted electronically at the links below. Please note, the links are for Memorial students and staff only–Jefferson students and staff can access their forms here.

Paper forms are available in the Student Services office. The federal government is providing the vaccine at no cost. If you are insured, please bring your insurance information on the day of the vaccine (the government requires Walmart to bill insurers for administering the vaccine when this is available). However, this is NOT required. You will not be billed for the vaccine or for the administration of the vaccine.
